A person named Keely discovered Rivers and reported finding him in a pool of blood after he had apparently fallen off the toilet. “Sam Rivers was found in the bathroom of his home by a person named Keely. Keely reported that ‘she found Sam in a pool of blood’ after Rivers had apparently fallen off the toilet,” the report stated.

The deputy’s account noted a cut over Rivers’ eye. “They say Sam appeared to have a cut over his eye, a possible result of a fall from the toilet.” The report also documented unusual physical indicators at the scene. “In the report, the deputy says Sam’s face and neck were dark blue, while the rest of his body was flushed… which they note is commonly seen in instances of pulmonary embolism.”

The tragic incident occurred on October 18, 2025, when the 48-year-old bassist was discovered in his Florida home. Rivers’ wife, Keely, found him and immediately called 911. She performed CPR in an attempt to save his life before first responders arrived at the scene, as reported by Perplexity research.
